If the original business account still exists, first try Microsoft’s work-or-school password reset at https://passwordreset.microsoftonline.com/; it can restore access directly when self-service password reset was enabled and the account has registered security information.^1^

The QR code and public form link normally provide respondent access only. They do not grant ownership, editing access, or access to responses.

If password reset does not restore access

Ask an administrator in the original Microsoft 365 organization (tenant) to handle the form. Your new business account cannot perform the recovery merely because it has the old form link.

The original tenant’s administrator should determine the account state:

    • Account active but sign-in restricted/locked: the administrator must restore sign-in or reset the account first. The Forms delegation page will reject an owner account that is still active.
    • Account disabled: an Office Apps Administrator with a valid Forms licence can transfer the form; there is no transfer time limit while the account remains disabled.
    • Account deleted within the last 30 days: an Office Apps Administrator can still transfer the form.
    • Account deleted more than 30 days ago (hard-deleted): the form and its responses cannot be recovered.^2^

    Administrator transfer procedure

An Office Apps Administrator in the original tenant should:

    1. Sign in using an administrator account that has a valid Microsoft Forms licence.
    2. Open this address, replacing the placeholder with the original form owner’s email address:
         https://forms.cloud.microsoft/Pages/delegatepage.aspx?originalowner=******@contoso.com

    3. If no forms appear, replace the email address with the original user’s Microsoft Entra Object ID and try again.
    4. Locate the wedding-list form.
    5. Select More form actions → Move.
    6. Move it to a Microsoft 365 group in the original organization. The administrator must be a member of that group.^2^

Microsoft’s normal Move operation only works for the form owner, while an administrator uses the delegation procedure for a disabled or deleted owner.^3^

This procedure transfers the form within the original organization. If your new business account belongs to a different Microsoft 365 tenant, it cannot directly take ownership through the delegation page. After recovery, copy the questions and export the responses as needed into a newly created form under the new account.

If nobody can administer the original tenant

If another Global Administrator exists, ask that administrator to restore your account or assign the required administrator access.

If you were the only Global Administrator and the entire original tenant is inaccessible, contact Microsoft support and request help with a tenant lockout / Microsoft 365 Data Protection case. Be prepared to verify control of the organization through information such as its domain and billing records.^4^

If the subscription was bought directly from Microsoft, an administrator can open the Microsoft 365 admin center, select Help & support, and create a request. If all subscriptions were purchased through a Microsoft partner, contact that partner instead.^5^

    When contacting support, provide:

    • The original account address and organization/domain.
    • The form title.
    • The approximate date access was lost.
    • Whether the account was disabled or deleted, if known.
    • The form response link or QR destination as an identifier—not as proof of ownership.
    • Any error message shown during sign-in or on the delegation page.

    Hi @Dave Tyler

Could you please confirm whether the original account used to create the form was a personal Microsoft account (such as Outlook.com or Hotmail) or a work or school account managed by an organization?

If it is a work or school account, an administrator may be able to help transfer ownership of the form, provided the requirements outlined in this article are met.

Since this was your wedding list, it's possible that you created the form using a personal Microsoft account rather than a work or school account. If that is the case and the account has been closed, restricted, or is no longer accessible, there is unfortunately no supported way to transfer ownership of the form to another account using only the form link or QR code. The owner would normally need to sign in and share the form or add a co-owner.

In that situation, your best option would be to try to recover access to the original Microsoft account using Microsoft recovery form. If account recovery is not possible, access to the form and its responses may not be recoverable.
