I found an answer elsewhere. The issue is with Mac Privacy & Security. I needed to give OneDrive full file access. Hopefully that sticks and is the full answer.

There is no OneDrive setting that opens every PDF without restriction; first open an affected file on OneDrive.com to determine whether the problem is its local download state, macOS, or the PDF’s permissions.

    1. Test the PDF outside the OneDrive Mac app

    1. Select the OneDrive cloud icon in the macOS menu bar.
    2. Select Help & Settings → View online.
    3. Sign in with the same account used by the OneDrive app.
    4. Select an affected PDF:
      • If it opens online, download it and open the downloaded copy in Preview or Adobe Acrobat Reader.
      • If the downloaded copy opens, continue with the Files On-Demand step below.
      • If neither the online nor downloaded copy opens, note the exact message and use the permissions section below.

This online test is also Microsoft’s recommended first check when troubleshooting OneDrive sync on a Mac.^1^

    2. Keep the PDFs downloaded on the Mac

OneDrive Files On-Demand can leave files online-only. Such files require an internet connection and an active OneDrive sign-in before macOS can open them.

    In Finder:

    1. Open the OneDrive folder.
    2. Control-click the folder containing the PDFs.
    3. Select Always Keep on This Device.
    4. Wait until the download/status indicator shows the files are locally available.
    5. Open a PDF from Finder.

On macOS 12.1 and later, Files On-Demand cannot be disabled, but individual files or folders can be marked Always Keep on This Device.^2^

If the message says “Need permissions” or “Protected PDF”

That is not a OneDrive setup restriction that can be switched off. A PDF protected by a sensitivity label or rights management can be opened only by an account explicitly authorized for it, and its protection can override ordinary OneDrive sharing permissions.^3^

For a shared work or school file:

    1. Sign in using the exact work or school account to which the file was shared.
    2. Ask the file owner to grant that address access under the PDF’s sensitivity-label or encryption permissions.
    3. If necessary, ask the owner to provide an unprotected copy, subject to their organization’s policy.

If opening the PDF freezes rather than showing an error

If the Mac runs macOS 15.0 or 15.1 and the PDFs are in Desktop or Documents, update to macOS 15.2 or later. Microsoft identifies that opening/saving freeze as fixed in macOS 15.2.^4^

    Until updating:

    1. Open Applications → Utilities → Activity Monitor.
    2. Search for OpenAndSavePanelService.
    3. Select it, click X, and choose Force Quit.
    4. Move the PDF out of Desktop or Documents to another folder.
    5. Open it again.

If files open online but still will not download or open through Finder

    Repair the OneDrive connection:

    1. OneDrive cloud icon → Help & Settings → Preferences → Account.
    2. Select Unlink this Mac.
    3. Open Keychain Access, search for OneDrive, and delete OneDrive Cached Credential.
    4. Open OneDrive through Spotlight and sign in again.
    5. When prompted that the OneDrive folder exists, select Use this folder.
    6. Allow synchronization to finish, then mark the PDF folder Always Keep on This Device again.

Unlinking and relinking does not delete cloud files, but changes made while disconnected synchronize after relinking.^1^

If the problem remains, provide the exact error message, macOS version, OneDrive version, and whether the account is Personal or work/school. Personal-account users can contact Microsoft Support; work/school users should contact their organization’s IT administrator.^5^
