Even for a single-user subscription, Microsoft creates an “organisation” for the Microsoft 365 account, and in your case, you seem to be also the administrator since you are the one who paid for the subscription. Since the message in the screenshot said Microsoft Forms is currently not enabled for the account, and access to Outlook has also been lost, the subscription or license may be inactive or not assigned correctly.
Please try the following steps:
- Open a private or incognito browser window and go to Microsoft 365 Admin Center.
- Sign in with the business account used to purchase the subscription. If the Admin Center cannot be opened, try the original
onmicrosoft.comadministrator username created during sign-up. - Go to Billing > Your products and check that Microsoft 365 Business Basic shows as Active.
- Next, go to Users > Active users > select your account > open Licenses and apps.
- Confirm that the Microsoft 365 Business Basic license is assigned and that Microsoft Forms and Exchange Online are enabled.
- Save any changes, sign out, and then try Outlook and Forms again.
