Unfortunately, the only option is to contact Microsoft Rewards Support, as they are the only ones who are capable of investigating your suspension case.
However, since you mentioned that you are unable to submit a ticket because you cannot fill in the required fields on the form, I’m afraid your Rewards account suspension is permanent.
In this case, there is nothing that can be done.
