A web-based tool in Microsoft 365 that enables users to quickly create surveys, quizzes, polls, and feedback forms.
It’s good that you’ve recovered your account. The form may still have a separate restriction, since Microsoft Forms can block forms from collecting responses when its security checks flag them.
Here are some suggestions you can try:
Sign in with the account that created the form and open it for editing. Check for a warning about the block. If it identifies flagged wording, review and remove any unnecessary requests for sensitive information. Removing flagged keywords can lift a potential-phishing block.
I’d suggest testing the form’s response link directly in your browser, rather than scanning the QR code, to check whether the same message appears.
If you used a work or school account, ask your IT administrator to review and unblock both the form and any Forms-specific restriction on your account. These are separate checks.
Was the form created using a personal account, such as Outlook.com, or a work/school account? Please also share the exact blocked message, without posting account details.
