My Power Automate request can't be display from "Teams Approvals"

When users submitted a quiz via power automate, my "Teams Approvals" can display the application information and it said "Something went wrong. Wait a bit, then try again. CorrelationId - 774bded0-92cc-11f0-9687-8b17de6c2ae6". Please see screenshot as below.

    The error message in Teams Approvals usually indicates that the approval record exists, but Teams is unable to retrieve or render the underlying approval details.

    Here are some troubleshooting steps you can try:

    Verify the approval still exists

    • Open Power Automate > Approvals and check whether the approval request is present and accessible.
      • Confirm that the flow completed successfully and that the approval action wasn't deleted or modified after submission.
      Check the flow run history
      - Go to **Power Automate** > **My flows** > **Run history**.
      
         - Review the run that generated the approval and look for any failed or skipped actions.
      
            - Note any error details associated with the approval action.
      
            **Test in different clients**
      
               - Open the approval from:
      
                     - Teams desktop app
      
                           - Teams web app
      
                                 - Power Automate Approvals center
      
                                    - If it works in one client but not another, the issue may be related to the Teams client cache.
      
                                    **Clear the Teams cache**
      
                                       - Sign out of Teams.
      
                                          - Clear the Teams cache on the affected device.
      
                                             - Sign in again and retest.
      
                                             **Review permissions**
      
                                                - Ensure the users viewing the approval still have access to the related Microsoft 365 resources, forms, or data sources referenced by the flow.
      
                                                **Gather diagnostic information**
      
                                                   - The provided Correlation ID: `774bded0-92cc-11f0-9687-8b17de6c2ae6`
      
                                                      - If the issue persists, provide this Correlation ID, the approximate timestamp, and the flow run ID when opening a Microsoft support case.
