Artifact Signing organization identity validation failed after successful Verified ID — how to find the reason?

We are an Italian company (S.r.l., more than 3 years of fiscal history) and submitted a Public Trust organization identity validation in Azure Artifact Signing on 23 September 2026, using our VAT number as business identifier and the registered office address from the Chamber of Commerce. On 30 September the legal representative completed the Verified ID step in Microsoft Authenticator ("Result Successful"). Nine minutes later we received a "Validation Fail" email with no reason, and the portal now shows the request as Failed.

We understand a failed request cannot be reopened and that attempts are limited, so we would like to fix the cause before submitting one new request. Could someone from the validation team tell us which detail could not be verified (organization name, business identifier, address, website/domain, legal representative)? We can share the validation ID and subscription ID privately.
