My Form is being blocked - phishing

Hi, I'm a travel agent and created a form to give to my clients to collect information for their upcoming travel. I am not asking for any personal information other than names, email addresses, etc. I am not asking for confidential credit card or banking information either. My form is being blocked for potential phishing violation and I'd like to get it reviewed to see what those violations are. Can someone assist?

Hi, I inserted your problem to Google and this is the answer it gave regarding the reasons: Microsoft's proactive scanning algorithms do not give specific error descriptions, instead they dynamically flag patterns that look like identity or credential harvesting.

What comes to possibly solving the issue, it gives this answer: If you use a Work, Business or Host Agency Account, you need your Microsoft 365/IT Administrator to review/unblock it. If you use a Personal Microsoft Account it says that Microsoft does not provide a manual review or appeal system for personal accounts facing automated form blocks. Your best approach is to bypass the filter, in other words, you need to re-word it.
