This happens to me all the time now. If I don't deliver one command right after the other it freezes up when I say the first command. If I say the command again it'll activate it. Also why isn't my other message posted is it waiting for approval?
Anyone else experiencing problems with Voice Access after last week's update?
Everytime I try to dictate a sentence using voice access with classic outlook (and other apps) it says "working on it" and I have to keep closing and reopening.
How can you raise this as an issue with Microsoft?

Yes, this "working on it" freeze getting stuck has been reported by others after recent updates, so you're not alone here. A few things worth trying first before escalating:
Check Settings > Windows Update for any pending Voice Access or Windows 11 updates - Microsoft has been pushing incremental fixes for accessibility features, so there may already be a patch for this. Also worth restarting the Voice Access service itself rather than the whole app: turn it off and on from the Accessibility settings page (Settings > Accessibility > Speech > Voice access) rather than just closing the floating bar.
If it's still hanging afterward, the best way to actually get this in front of Microsoft (rather than just the community here) is through the Feedback Hub - open it, go to Accessibility as the category, and search first to see if others already logged the same "working on it" freeze so you can upvote it instead of duplicating. If nothing matches, submit a new report with the recreate steps you gave here (classic Outlook, dictating a sentence, freeze) and attach a screen recording if you can, since that gets picked up by the product team directly and carries more weight than a community post.
